Our Long-Term conditions (LTC) theme delivers research that directly improves health outcomes, reduces inequalities, and strengthens the sustainability of care systems across the North West Coast and beyond.

We focus on the growing challenge of longterm physical and mental health conditions, including multimorbidity and complex care needs. Our work supports system partners by generating evidence that enables:

  • Earlier prevention and reduced disease progression, helping services manage demand more effectively.
  • Higherquality , more coordinated care, improving service efficiency and patient outcomes.
  • Better lived experience, leading to improved wellbeing, independence, and engagement with services.

Impact driven research

We develop and evaluate practical solutions such as selfmanagement programmes, personalised care approaches, and communitybased support models that can be scaled across health and care systems. Where appropriate, we assess the role of digital tools in improving access, monitoring, and continuity of care.


Reducing inequalities

Addressing health inequalities is central to our mission. We focus on communities facing the greatest disadvantage, generating evidence on what works in realworld settings and identifying systemlevel changes that can narrow outcome gaps. This helps partners target resources more effectively and deliver fairer, more responsive services.


Co production and system alignment

Our research is developed in partnership with patients, carers, community organisations, and frontline professionals. This ensures that our work reflects real needs, enhances local capacity, and supports decision makers with evidence that is usable, credible, and aligned with regional priorities.
